This case-control study used data from the National Institute of Neurological Disorders and Stroke-sponsored, Multicenter Study of Clinically Significant Cerebral Arteriovenous Malformations. Study subjects were 1140 adult patients with a diagnosis of a clinically significant cerebral arteriovenous malformation, who were consecutively enrolled from 13 medical centers between January 2003 and August 2008. Cases were hospitalized patients who experienced sudden death due to a noncardiac cause during the study period. For each case, two age- and sex-matched controls were randomly selected from those enrolled within the study period. Antipsychotic drugs were ascertained using the medical records of the study subjects. Unconditional logistic regression was used to estimate the odds ratios (ORs) of death from noncardiac causes associated with antipsychotic use. In total, 79 subjects (6.7%) experienced sudden death during the study period. Of these, 31 (39.2%) were taking antipsychotics at the time of death. The OR for sudden death in the antipsychotic group was 2.3 (95% confidence interval, 1.2-4.3). The most common antipsychotic drugs were haloperidol (n = 20), thioridazine (n = 12), and clozapine (n = 6). The ORs of sudden death in patients taking haloperidol and thioridazine were 4.2 (95% confidence interval, 1.4-12.5) and 3.0 (95% confidence interval, 1.1-8.2), respectively.
